Sex reassignment surgery without vaginoplasty
Dr. Chettawut offers a more conservative surgical approach than the standard SRS which is called the cosmetic sex reassignment surgery (cosmetic SRS) to provide more options for male to female prospective SRS patients based on their personal preferences.
In terms of the external appearance, the aesthetic result of the cosmetic SRS is similar to the standard SRS because Dr. Chettawut performs both procedures by using the same non penile inversion technique.
What makes the cosmetic SRS surgery different from the standard SRS is that it does not include the creation of a vaginal canal so there will be no functional depth for penile penetration.
Cosmetic SRS consists of elimination of male genital organ (removal of the penile shaft and testes), creation of all external female genital structures which are clitoris, clitoral hood, labia major and minora, reconstruction of the vulva vestibule, urethral and vaginal opening.
A shallow vaginal opening (1.5 - 2 inches depth) is needed to maintain the natural look of female genitalia and can possibly make an illusion that it has a vaginal canal.
The urethral opening is created in the anatomical position (same as the biological female’s urethra) for normal urinary function and according to Dr. Chettawut’s SRS technique, the corpus spongiosum (the male spongy tissue around urethra) will be totally discarded to prevent engorgement of the spongy tissue during sexual arousal.
Sensitivity and orgasm are undoubtedly achievable with cosmetic SRS as the nerves responsible for sensation are carefully preserved during the surgery.
Comparison between the standard SRS and the Cosmetic SRS
Operation time and Recovery
Since there is no vaginal construction, the operation time of the cosmetic SRS can be reduced to 4 hours (the operation time of standard SRS by Dr. Chettawut’s non penile inversion technique is averagely 6 hours).
The cosmetic SRS patients can be expected to have faster recovery because the vaginal dilation routine is no longer required.
Possible risk or complication
In cosmetic SRS, there is no creation of vaginal tunnel between the rectum and the urethra (beginning part of vagina) or between the rectum and the bladder (deep part of vagina) so it significantly lowers the risks/complications related to the injury of the above mentioned organs.

Prospective SRS patients who do not wish to have a functional vagina for the use of sexual penetration, the cosmetic SRS should be taken in consideration.
