Dr.Chettawut's answers for your questions regarding facial cosmetic surgery/ body contouring surgery

1. Buccal fat pad
Question:   What is Buccal Fat Pad Extraction (Cheek reduction surgery)?  and how is the operation done?

Answer: Buccal fat extraction or cheek fat removal is the procedure to remove the fat pads that augment the lower part of the cheeks.

I will make incisions about 2 cm long between the cheek and gums, starting above the second upper molar towards the back of the mouth to expose the fascia of the buccinator muscle (cheek muscle).

While applying the pressure externally against your cheek right under your cheek bone, the buccal fat can be easily protruded through the incision and the appropriate amount of fat pad will be meticulously removed.

2. Crow feet's reduction
Question:   What is the surgical correction for crow's feet? and how is the result?

Answer:   This is the surgical procedure to reduce dynamic lines around eyes (crow's feet).

The procedure involves partial removal of the muscle ring at the tail of your eyes with fascial graft interposition via the temporal incisions or face lift incisions (in case done together with face lift surgery).

The procedure can permanently reduce about 60-80% of crow feet (long term correction).

3. Breast lift surgery
Question:   I have breast sagging and would like to have breast lift surgery done, what is the appropriate technique for me?

Answer:   There are three techniques for breast lift/reduction, depends on the severity of breast tissue and skin sagging.

1. Round block technique, this is only for moderate size and mild degree of sagging. There is only circular scar around the nipple-areolar area.

2. Vertical scar mastopexy, I prefer this techniqe for most of my patients. There are circular scar around the nipple-areolar areas plus the vertical scar extend to the lower fold of breasts.

3. Inverted T technique, this is for huge breast and the scar will be like inverted T (vertiacl scar from areolar area to the mammary fold plus the horizontal scar along the fold)

4. Protruding abdomen
Question:    I would like to have only liposuction to avoid scar from tummy tuck, can I do that?

Answer:    Yes, if you have only local fat deposit problem in your belly.

Anyway, if you also have excess skin and laxity of abdominal muscle, you can get a better resi;t from the tummy tuck procedure, not the liposuction.

5. Lip reduction surgery.
Question:   I would like to correct my protruding lip, can you explain me more about the procedure?

Answer:    Yes, the lip reduction procedure will be done on the lip mucosa (vermilion) inside your month with local anesthesia.

If the dry vermilion is not excessively large, the reduction surgery is designed as a transverse ellipse behind the wet line. However, if the entire vermilion is enlarged, then design of the excision may include the dry vermilion.

The wedge-shaped excision (at least 5 mm.) removes mucosa, submucosa, submucosal glandular tissue, and occasionally underlying muscle. The future scar will be inconspicuous.
