Concept of modern Sex reassignment surgery

1. Achieve very good sensation on the erotic sensate area, especially neoclitoris, clitorlal hood and labia minora.

Most of my patients (95%) can get orgasm within one year after surgery.

2. Normal or nearly normal look like natal female genitalia.

These emphasize on the form of:

- vulva (outer labia and vaginal opening)

- clitoris and its hood

- well defined inner labia (thin stretches of tissue within the labia majora).

3. Functional vaginal depth which should obtain at least 5 inches in normal cases.
My average is 6 inches (15cm). For those whose large frame with deep rectovesicle pouch, the depth of 7 inches or more is possible.

Factors influence the depth of your new vagina

Your anatomy: The plane of dissection for newly created vaginal tunnel during SRS is between the rectum and urethra/ prostate/ bladder thourgh the pelvic cavity.

The depth of your neovaginal should be expected between 5-7 inches if you have normal anatomy and no inflammation or scar along the plane of dissection.

The skin graft availability:

The skin graft must be adequate for the lining inside neovagina, which normally come from only scrotal skin graft (90% of my cases). Only 10% whose scortal skin is not enough, the abdominal skin graft will be needed as the extra procedure to obtain a functional vaginal canal.

Postoperative dilatation: The final outcome will also depend on your responsibility to maintain the depth or even get more depth by diligent dilatation.

Candidates for male to female Sex reassignement surgery

Once a diagnosis of transsexualism has been made, recommendation for SRS should be based on strict criteria as laid down by The Harry Benjamin International Gender Dysphoria Association (1985) as follows:

1. The patient should show evidence of stable transsexual orientation

2. The patient should show insight into his/her condition and should not suffer from any serious psychiatric disorder

3. The patient should be able to pass successfully as a member of the opposite sex, and there should be clear evidence of cross-gender functioning

4. Improvement in personal and social functioning should be predicted for the individual prior to and after surgery.

Criteria for patients scheduling sex reassignment surgery

The patients must legally reach maturity age in the patient's nation.

The patients must be strongly persistent in their desire for sexual reassignment surgery at least 2 years.

The patients must have lived and worked exclusively in the cross-gender role for not less than 6 months.

The patients must have female hormonal and/or anti-androgen treatment (HRT) for not less than 6 months before the surgery.

The diagnosis of gender dysphoria must be made by a psychiatrist, clinical psychologist, or endocrinologist/sexologist dealing with transsexuals.

Required documents for scheduling SRS

I require at least one referral letter for SRS recommendation by a psychiatrist, clinical psychologist, or endocrinologist/sexologist who is a professionally qualified specialist in the field of gender dysphoria.

At the time of consultation, I myself will make a final decision regarding your eligibiligy.

If your eligibility criteria for sex reassignment surgery are in doubt or you can not supply referral for SRS , you may need to consult with psychiatrist here on a one-time basis, for which she charges 300 US$.

Required preoperative blood and laboratory tests

1. CBC (complete blood count and platelet count)
2. Anti HIV
3. BUN & Cr (kidney function test)
4. PT & PTT (screening test for bleeding tendency due to liver disease)
5. FBS (fasting blood sugar)
6. Chest x-ray
The electrocardiogram (EKG) is also required if you're over 45 yr.

All medical check up should be done at 1 month before surgery and fax me when you get the reports.

Age/ Weight limitation for my SRS patients (related to height)

For incoming patients, please note that you must control your weight to be acceptable according to the email correspondence, otherwise your surgery might be postponed.

Scheduling sex reassignement surgery with facial feminization surgery

For those who plan to have sex reassignment surgery done together with a set of major facial feminization surgeries in one trip, please note that I normally separate SRS from FFS with some day interval to minimize the total time for general anesthesia.
